Middle School Sports Physical Form

Physicals are required for all 7th and 8th graders wishing to play sports, however, the school district has taken back all 7th and 8th grade sports except soccer and lacrosse. For all other sports, please send your physicals to them.

Tent Rental Agreement

Please be aware that our rates and policy has changed for the 2019-20 fiscal year. To receive the nonprofit rate, you must be able to provide proof of nonprofit status.
